In this role, you'll provide in-home evaluation and therapy for clients with physical, emotional, or cognitive limitations. You'll help patients achieve greater independence through customized care plans, family education, and goal-oriented therapy interventions.What You'll Do

• Conduct home-based assessments within 72 hours of referral (unless otherwise specified).
• Design and implement individualized treatment plans focused on improving daily living skills and function.
• Educate patients and caregivers on therapy techniques, home exercise programs, and insurance guidelines.
• Participate in interdisciplinary case conferences and collaborate across care teams.
• Maintain clear, accurate documentation in compliance with all home health guidelines and regulations.
• Ensure care is age-appropriate across the lifespan, from infants to geriatrics.What You'll Need
  • Bachelor's degree in Occupational Therapy required (Master's preferred).
  • Current Iowa Occupational Therapy License (provisional license with supervision accepted for new grads).
  • Valid Iowa driver's license, proof of insurance, and reliable transportation.
  • BLS certification and Mandatory Reporter certification.
  • 0-2 years of experience in occupational therapy (home health experience preferred).
