for a skilled Home Health Physical Therapist to provide dedicated
and compassionate service to help our clients achieve their highest
level of mobility and independence. This role is significant to our

therapy management tailored to individual patient needs. The

bonus. Conduct patient assessments to determine their physical
condition and establish therapy goals. Create and implement
detailed treatment plans tailored to each patient’s needs and
abilities. Document patient progress and adjust treatment plans as
necessary. Educate patients and their families on exercises and
treatments to aid recovery at home. Work collaboratively with other
healthcare professionals to ensure comprehensive patient care.
Adhere to all professional and ethical standards, including patient
confidentiality and rights. Expertise in developing and
implementing physical therapy treatment plans Strong interpersonal
and communication skills Ability to work independently and as part
of a multidisciplinary team Proficiency in using physical therapy
equipment and software Medical Insurance Dental Insurance Vision
Insurance 401k Valid Physical Therapist State License CPR/BLS
Certification Proof of current physical and TB test (if required by
state regulations)
