Pharmacist Location: Mauston, WI Agency: Ambition 24Hours Pay: Competitive weekly pay (inquire for details) Shift Information: Days - 5 days x 8 hours Contract Duration: 7 Weeks Start Date: ASAP About the Position Physical Therapist (PT) Mauston, WI | Rehabilitation Therapy | Permanent | Full-Time | Day Shift Job Description The Physical Therapist (PT) is responsible for evaluating and treating patients with mobility limitations, movement dysfunction, or pain in an acute or post-acute setting. The PT will develop and implement individualized treatment plans to improve functional mobility, support recovery from surgery, and help prevent adverse events such as falls and deep vein thrombosis (DVT). The role includes timely and accurate documentation, effective communication with the interdisciplinary care team, and active participation in discharge planning and care coordination. Key Responsibilities Evaluate and treat patients with movement dysfunction, mobility limitations, or pain. Develop and implement individualized treatment plans to improve functional mobility and independence. Assist patients in recovery from surgery and support prevention of adverse events (e.g., falls, DVT). Document evaluations, daily treatment notes, progress notes, and discharge summaries in a timely manner. Communicate effectively with physicians, insurance providers, patients, families, and rehab staff. Participate in discharge planning and care coordination as part of a multidisciplinary team. Maintain cleanliness and organization of treatment areas and assist with routine departmental tasks. Required Qualifications & Skills Degree in Physical Therapy from an accredited program. Eligible for or holding an active Wisconsin Physical Therapist (PT) license (or ability to obtain upon acceptance). New graduates welcome (less than 1 year of experience acceptable). Strong documentation and written/verbal communication skills. Ability to work independently and collaboratively within a multidisciplinary team.
