Overview:As a member of an interdisciplinary healthcare team,
the Hospital Pharmacist works with providers to determine optimal
therapy and dosing of medications while guiding nurses and other
clinical staff on administration, interactions, compatibilities,
and side effects. The Pharmacist provides safe, accurate and
optimal medication regimens throughout the inpatient stay while

Responsibilities:Product VerificationEnsures medication being
dispensed is accurate and in date.Ensures provider orders are
checked for appropriate dosing, interactions, and overall
appropriateness.Ensures medications are secure and not accessed by
individuals who shouldn't have access.Clinical ServicesProvides
accurate admission medication reconciliation.Provides dose
adjustments, route changes, and antibiotic
recommendations.Participates in groups and or committees relating
to medications.Development and EducationEducates patients and staff
on medications or medication classes.Provides continuing education
within the department to enhance staff knowledge.Participates in
the pharmacy college student rotation program.Ensures proper
storage, preparation, transport, and disposal of medications within
the hospital.Delegates and provide direction to pharmacy
technicians in the absence of the Director of
Pharmacy.Qualifications:Minimum RequirementsIdentify items that are
minimally required to perform the essential functions of this
position.Preferred or SpecializedNot required to perform the
essential functions of the position.Education:Bachelor of Science
in Pharmacy or PharmD from accredited College of PharmacyCompletion
of a hospital-based or other applicable residency
programExperience:1 year of prior hospital
experienceLicense(s)/Certification(s):Current licensure in good
standing with the Iowa Board of PharmacyValid driver's license when
driving any vehicle for work-related
reasons.Knowledge/Skills/Abilities: Writes, reads, comprehends and
speaks fluent English. Microsoft Office - basic skills.
Customer/patient focused. Critical thinking skills using
independent judgment in making decisions.Other:Use of usual and
customary equipment used to perform essential functions of the
