This care will be determined by individualized plans of care, care guidelines, best practice data and critical paths.What you will Do: - - - - - - - - - - Mobilizes resources and manages the systems to achieve outcome-based care - - - - - - - - - Coordinate physician, patient and payer interactions to ensure appropriate level of care. - - - - - - - - - Respond to concurrent and retrospective data analysis identifying trends in patient care for that unit's patient population.
