Employment Type: Full time Shift: Day Shift Description: Position Purpose: The Case Management Nurse works with the interdisciplinary team for patients on a select unit from admission to discharge. This care will be determined by individualized plans of care, care guidelines, best practice data and critical paths. What you will Do: Mobilizes resources and manages the systems to achieve outcome-based care Coordinate physician, patient and payer interactions to ensure appropriate level of care. Respond to concurrent and retrospective data analysis identifying trends in patient care for that unit's patient population. Hours/Schedule: .9 fte, Days.Minimum Qualifications: Active IA RN licenseBSN preferredMinimum of 4 years of clinical experience.
